Job Description

I) Job Purpose

The Family Connects Registered Nurse (RN) provides nursing in a home visit setting to families with newborns in the area(s) assigned by the Family Connects Nurse Lead. The nurse supports and assesses families in 12 domains of physical and psychosocial well-being and facilitates referrals and connections to community agencies to support families. The nurse uses the nursing process in nursing assessments, planning, implementation/intervention, education, evaluation and consultation.   The nurse is a member of the interdisciplinary treatment team and works within the team environment to schedule, visit and support families using the evidence-based Family Connects home visiting protocol.

II) Essential Functions

A. Provides nursing assessment and nursing support. These are based on use of the nursing process for planning and identifying current or predicted health care needs for newborns and families served by Family Connects. 

Performance Standards

Follows the Family Connects scheduling protocol, including productivity requirements as outlined by supervisor.
Keeps records of client activities and documents assessments and visits in real time via in-home use of computerized record system.  All nursing progress notes conform to MHMR Tarrant standards as well as Medicaid standards.
Provides health and physical assessment of mother and baby in the home to determine well-being.  Assesses family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence. Exercises clinical skills in appraising situations and making decisions.
Establishes a trusting relationship with families during home visits and engage families to establish support plans with goals, objectives and activities that address family strengths and needs.
Applies working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assists in creating a health plan for entire family.
Works with Family Connects team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings as required. Supports and encourages development of other staff and Family Connects.
Maintains necessary supplies for implementation of agency policies and procedures on infection control, according to the Infection Control Manual.  Carries out measures to prevent exposure to and transmission of pathogens in accordance with agency standards on infection control.
 

B. Shows evidence of continuing professional growth.

Performance Standards

Completes all MHMR Tarrant agency training requirements within first 60 days of employment and will complete all annual training updates as required.
Become fully trained in Family Connects home visiting protocol as stated in the current program implementation manual.  This may initially include trips to Durham, NC, for observations, training and “certification”.
Attends in-services or continuing nursing education programs which benefits nurse’s knowledge and client care. These must be approved by the supervisor.  Provides a copy of the attendance roster or certificate for verification.
Maintains continuing nursing education units to comply with standards set by the Texas Board of Nursing, as evidenced by and proof of attendance.
Provides evidence of all outside training as well as any upgrades in credentials to supervisor, Training Department and/or Human Resources.
Attends scheduled nursing meetings as appropriate to facilitate networking, professional growth and information dissemination.

Family Connects home visiting protocol and assessment of 12 domains of physical and psychosocial well-being.
Familiarity with chronic mental health, integrated health and chronic disease populations and optimal care trends.
Knowledge of medications, their use and typical side effects.
Ability to distinguish between medical and nursing interventions.
Ability to advocate for the patient.
Familiarity with trends in substance use disorder treatment.
Effective time-management skills, organization, and multitasking skills.
Understanding and commitment to a universal home visiting program as a public health intervention/prevention for newborns and their mothers.
RNs will be required to retest on position specific competencies and undergo chart reviews annually to ensure safe practice for both the RN and clients we serve.
 

IV) Internal & External Customer Service

Knowledge, skills and ability to work effectively with staff and families/adults from various cultural, linguistic, socio-economic, cognitive, social-emotional and educational statuses.

Ability to problem solve with infrequent assistance from supervisor. Identify and use a rational approach for the situation. Communicate problems and decisions to supervisor.
Ability to discuss uncomfortable and potentially conflict-producing, stressful topics in a professional and effective manner with internal and external customers and maintains courteous interaction.
Communicates with clinical supervisor regarding complex clinical or ethical issues, each time they arise.

 

V) Travel

Travel within the area(s) assigned by supervisor to meet the needs of the newborns and families is required.

This position may require temporary or permanent re-assignment to any MHMR if Tarrant facility of program as determined by the program needs or the Division Chief, supervisor, Director or Nursing Director.

Required Education
Associates Degree  or higher

Required Experience

Defined Experience
Preferred: One year nursing, preferably with families with newborns or young children
Preferred: Clinical experience related to domestic violence, substance abuse, maternal depression, child/maternal health

Current Texas RN license
Valid Texas driver’s license with an acceptable driving record.
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate
