What will you be doing?
The purpose of the Client Navigator-Short Term Case Management is to assist Individuals and/or Families gain knowledge, skills, and tools to ensure client stability, progress and success.
Work collaboratively as part of a Case Management Team
You will ensure completion of client assessments and development of a service plan that is realistic and achievable.
Provide Financial Assistance (FA) and connection to resources
You will assess client needs and goals as well as coordination of services with internal and external resource providers.
Ensure professional delivery of service
You will ensure excellent rapport and communication with client and community partners. Complete all necessary documentation and reports, as well as participate in regular client staffing's with internal and external partners.
Regular office hours are 8:00 am to 5:00 pm, Monday through Friday. This position may require flexibility to work various hours and possibly weekends, depending on current program needs.

This is a full-time, non-exempt position with an hourly range of $17.00 - $19.00, based on skills and experience.
