Job Description
Recovery Resource Council is looking for a conscientious, detail-oriented, and compassionate individual who is capable of successfully connecting Veterans to VA health care benefits or community health services where Veterans are not eligible for VA care. The SSVF Health Care Navigator (HCN) must be able to provide case management and care coordination and work closely with the Veteran’s primary care provider and members of the Veteran’s assigned interdisciplinary treatment team.
 

Competencies, Knowledge, and Qualifications:

·Ability to work one on one or with large groups

·Ability to work independently and as part of an interdisciplinary team

·Ability to organize and prioritize work

·Skill in conducting client assessments

·Skill in formulating client goals and objectives

·Ability to write clear, concise case notes

·Strong communication skills including the ability to deliver presentations to a group

·Knowledge of substance use disorders and mental health issues

·Knowledge of community resources

·Ability to work flexible schedules including some nights and weekends

·Ability to communicate effectively, both orally and in writing

·Knowledge of community resources and ability to network

·Ability to plan, organize, set objectives, time frames, and prioritize

·Understanding of and willingness to practice trauma informed care

·Experience working with veterans is a plus

 

Skills / Requirements:

To perform this job successfully, an individual must have a Bachelor’s degree in a related field. Master level social worker or equivalent education preferred. Experience working with people experiencing homelessness and/or veterans is preferred. Must have reliable transportation, a current Texas driver’s license and evidence of auto liability insurance.
 

Responsibilities:

The Health Care Navigator (HCN) is responsible for conducting assessments with Veteran clients to understand their situation, potential barriers to care, the causes, and the impact of barriers on the Veteran’s ability to access and maintain health care services, as well as their strengths, limitations, risk factors, and supports. The HCN’s focus is on community services, outreach, and referrals while also working with the interdisciplinary team to develop a care plan. The HCN should provide care coordination and connection with services from other organizations and programs to assure services are complementary and comprehensive. The HCN continually evaluates the effectiveness of resources and referrals and makes modifications as needed. The HCN must also maintain thorough documentation and remain in communication with the treatment team members when appropriate. The HCN supports the veteran to meet their health goals and provides ongoing follow-up. The HCN also provides health education and materials to the Veteran and family members. The HCN must be able to work with and interact with Veterans from a variety of ethnic, cultural, and economic backgrounds, adhering to the agency guarantee to provide everyone with “unconditional acceptance, respect, and positive regard.”
